WE NEED YOUR HELP! Parents and volunteers are an important part of HYFL, as we are not sponsored by the Katy Independent School District. Your help in any way will be greatly appreciated. The following is a list of volunteer positions currently available. Please check one or more areas where you would be willing to participate.

Informed Consent / Risk of Injury: I /We, the undersigned as the parent or legal guardian of the child listed on this application, in consideration of this request give permission for my /our son /daughter to participate in the HYFL youth football /cheerleading program offered by HYFL (Houston Youth Football League). I /We understand and acknowledge by allowing my /our child to participate in this activity that the risk of injury and /or death may exist.

Medical Release: In order for the HYFL to accept the registrant for its football /cheerleading programs and activities, I /We hereby release, discharge, and /or otherwise indemnify the HYFL and its Board of Directors, sponsors, their employees, and any associated personnel including the owners of the fields and facilities utilized by the HYFL against any claim by or on behalf of the registrant as a result of the registrant’s participation in the Houston Youth Football League.

Conditions of Enrollment: By submitting the information on this form, I/We agree to the conditions stated above.

Refunds: All our sales are final and no refunds will be given. Once payment is received it is impossible for you to return a a uniform for a refund.There are no refunds for any hyfl sign-up. There are no refunds on uniform orders. Please review your choices carefully before proceeding to payment. There are will be no refunds or exchange because of typo or user error.
